A tragic incident in Ohio has left a family grieving after a 3-year-old boy accidentally shot himself at his home. His mother, Carley Dials, 29, has been arrested and charged with alleged involuntary manslaughter in connection to his death.

Authorities responded to the 6600 block of Kodiak Dr. near Canal Winchester just before 9 a.m. on Thursday, Sept. 18, following reports of a shooting. Officers found Cartier Smith with a gunshot wound and rushed him to the hospital, where he was pronounced dead within an hour, according to ABC6 and the Columbus Dispatch.

A preliminary investigation revealed that Smith accessed an unsecured handgun inside the residence and unintentionally shot himself. Police stated in a press release that the handgun was accessible due to the actions of Carley Dials.

According to the complaint filed by detectives, Dials had taken a handgun belonging to Sean Perrin, 31, off the bedroom floor and placed it between the mattress and the wall before going to bed, the Dispatch reported. At some point, the boy entered the room, found the gun, and accidentally fired it.

Perrin is facing a weapons charge, ABC6 reported.

Dials remains in custody at the Franklin County Jail, with bond set at $50,000, according to the Dispatch.
