A tragic accident in Ohio has left a 3-year-old boy dead and his mother facing serious charges. Carley Dials, 29, was arrested for alleged involuntary manslaughter after her young son accidentally shot himself at the family’s home.

Police were called to the 6600 block of Kodiak Dr. near Canal Winchester for a reported shooting just before 9 a.m. on Thursday, Sept. 18. Officers found Cartier Smith suffering from a gunshot wound and rushed him to the hospital, where he was pronounced dead less than an hour later, according to ABC6 and the Columbus Dispatch.

A preliminary investigation revealed that Smith had accessed an unsecured handgun inside the home and accidentally shot himself. Authorities stated in a press release that the gun was accessible due to Dials’ actions.

According to the complaint, Dials had taken a handgun belonging to Sean Perrin, 31, from the bedroom floor and placed it between the mattress and the wall before going to bed, the Dispatch reported. At some point, the boy found the gun and unintentionally discharged it.

Perrin is facing a weapons charge, ABC6 reported.

Dials remains in custody at the Franklin County jail with a bond set at $50,000, according to the Dispatch.
