A six-vehicle collision unfolded on a busy highway in Malaysia’s capital after a toddler reportedly opened a car door mid-journey.

The incident occurred on Friday, Feb. 6, at approximately 6:15 p.m. local time. A Honda City was traveling along Jalan Tun Razak in Kuala Lumpur when a 2-year-old child, seated on a relative’s lap, inadvertently opened the rear left passenger door, according to reports from Singaporean outlet AsiaOne and Malaysian outlets The Star and The Sun.

Authorities indicated that the vehicle’s child safety lock may not have been engaged.

“It is believed that the child safety lock feature was not activated, allowing the door to be opened from inside,” Mohd Zamzuri Mohd Isa, chief assistant commissioner of the Kuala Lumpur Traffic Investigation and Enforcement Department, said in a statement to The Sun.

Police said a 25-year-old motorcyclist was unable to avoid the suddenly opened door and collided with it, losing control and triggering a chain reaction involving multiple vehicles. A second rider, aged 30, who was traveling behind him, was also unable to avoid the crash.

The 25-year-old motorcyclist sustained a broken left arm and was taken to a local hospital for treatment. The older rider suffered minor injuries. No injuries were reported among occupants of the other vehicles involved.

Video footage capturing the collision has circulated online.

The case is currently under investigation for inconsiderate and careless driving under the Road Transport Act, AsiaOne reported.
