The Department of Defense has identified six U.S. Air Force members killed Thursday when a KC-135 Stratotanker crashed in western Iraq, marking one of the deadliest aviation incidents since the escalation of hostilities between the U.S.-led coalition and Iranian-backed forces began last month.
The crash occurred on March 12 during Operation Epic Fury, a high-stakes aerial campaign launched in response to regional instability. While the cause of the incident remains under investigation, U.S. Central Command (CENTCOM) confirmed that neither friendly nor enemy fire was a factor. The fatal crash involved one of two refueling aircraft operating in tandem; the second aircraft landed safely.
The Fallen Airmen
The victims represented two major refueling wings, highlighting the critical role of logistics in the current theater of operations.
From the 121st Air Refueling Wing (Rickenbacker Air National Guard Base, Ohio):
- Tech. Sgt. Tyler H. Simmons, 28
- Capt. Seth R. Koval, 38
- Capt. Curtis J. Angst, 30
From the 6th Air Refueling Wing (MacDill Air Force Base, Florida):
- Maj. John A. Klinner, 33
- Ariana G. Savino, 31
- Tech. Sgt. Ashley B. Pruitt, 34
The loss has reverberated across the military community. Ohio Governor Mike DeWine issued a statement Saturday praising the “elite airmen” for their service in “long-distance missions in defense of our nation.”

Families Remember the Fallen Amid Grief and Anger
As the identities were made public, families began sharing the personal tolls of the tragedy. Among them was Tech. Sgt. Tyler H. Simmons, a 28-year-old who relatives say had dreamed of flying since childhood.
“He had his dream job, for sure,” his grandmother, Bernice Smith, told NBC affiliate WCMH. “He loved the Air Force.”
Simmons was reportedly on the verge of a major career milestone. His cousin, Stephan Douglas, noted that following this assignment, Simmons would have secured the credentials necessary for promotion to Master Sergeant. However, the pride in Simmons’ service was tempered by sharp criticism of the ongoing conflict. Douglas described the war as “uncalled for,” echoing a growing sentiment of frustration among military families as casualties mount.
In Florida, the loss of Maj. John A. Klinner has left a young family shattered. Klinner, a pilot who was promoted in January, had been deployed for less than a week before the crash. He leaves behind his wife, Libby, and three young children, including seven-month-old twins.

Strategic Context and Investigation
The KC-135 Stratotanker is the backbone of U.S. aerial refueling, allowing combat jets and transport planes to remain airborne for extended durations. The loss of an entire crew underscores the inherent risks of “Operation Epic Fury,” which has seen at least 13 American service members killed since U.S.-Israel strikes on Iranian targets commenced last month.
The Air Force has grounded several training missions in the region as a precaution while investigators examine the wreckage in western Iraq. The Ohio Air National Guard Public Affairs office has not yet provided specific details regarding the flight’s final moments.
