A 28-year-old man from Massachusetts has died following an accident at a millwork studio.

David Morais of West Roxbury was moving a stack of plywood at Materia Millwork in Avon on Tuesday, Sept. 23, around 2:56 p.m. local time when the incident occurred, according to press releases from the Norfolk District Attorney’s Office and the Avon Police Department (Avon PD).

First responders from the Avon PD and the Avon Fire Department arrived following a report of a “fatal industrial accident,” per a fire department statement. Authorities found Morais inside the business, suffering fatal injuries, and he was pronounced dead at the scene.

The Massachusetts State Police, U.S. Occupational Safety and Health Administration (OSHA), and the Office of the Chief Medical Examiner were called in to investigate, the statement noted.

PEOPLE reached out to the Avon PD, Avon Fire Department, Massachusetts State Police, and the state medical examiner’s office for comment on Friday, Sept. 26, but did not receive immediate responses.

In an Instagram statement on Sept. 24, Materia Millwork described their grief over the “tragic accident.”

“David was more than a valued member of our team. He was a friend, a craftsman, and someone we were lucky to know. Our thoughts are with his family and loved ones during this incredibly difficult time,” the statement read.

“The safety and well-being of our team will always be our top priority. Out of respect for the ongoing investigation, we’re unable to share further details right now,” the company added.
