State authorities are investigating a fatal single-vehicle accident that claimed the life of a 27-year-old Master Sergeant on the grounds of the Louisiana State Penitentiary on Wednesday.

The Louisiana Department of Public Safety and Corrections (DOC) confirmed that Master Sergeant Tyler Grezaffi died following the incident at the facility, commonly known as Angola. Louisiana State Police (LSP) responded to the prison property on March 18 after receiving reports of a crash involving a lone vehicle. Grezaffi sustained fatal injuries at the scene.

While the LSP investigation remains active, preliminary reports suggest a delayed recovery. Sources indicated that Grezaffi’s vehicle veered off the roadway and submerged in a water-filled ditch. DOC officials were reportedly unable to locate the vehicle immediately; a deputy from the Pointe Coupee Parish Sheriff’s Office eventually discovered the wreckage approximately one hour later.

Grezaffi, a second-generation law enforcement officer, joined the DOC just over a year ago. She previously served as a deputy in both Pointe Coupee and Iberville Parishes, following the path of her father, Sgt. Scott Grezaffi.

“Although Tyler Grezaffi was at the beginning of her promising career in public safety and corrections, she had already made a significant impact,” said Gary Westcott, Secretary of the Louisiana DOC. Westcott praised her professionalism and the “unforgettable mark” she left on her colleagues since joining the department.

The Pointe Coupee Parish Sheriff’s Office (PCPSO) expressed profound grief over the loss of an officer they had watched “grow up” within their agency family. On Thursday, the sheriff’s office shared footage of a law enforcement procession transporting Grezaffi’s body, honoring her as a “bright spirit” and a dedicated public servant.

The Louisiana State Police have not yet released the specific cause of the crash as they continue to reconstruct the events leading up to the vehicle leaving the road.
